Why should you be unbanned: 

I don't believe I've done anything wrong except the use of a card that is not in my name. But I know the person of the card (My Mother) I'm using which I had consent from to use. Due to me getting a new card that hasn't arrived. Which isn't even wrong. If you have banned me for something else is quite bullshit since I've made 3 comments on various posts on the forums that weren't offensive in anyway. I've spent over $200 on scripts. And I can't even use them due to being banned. Actually, very annoyed at the wait time for a response to this appeal considering I haven't done anything wrong.

 

Below you will find evidence that I have access to the card that I have used. I'm really not sure how else to prove it and I'm not willing to show anything else.
